Possibly it is costly to cast and not easily used when no such danger is expected. There’s all kinds of ways, magic could be explained. Maybe Eldor is seriously reducing his capabilities while upholding this spell on her?

I think Eldor added the “attack with rainbow knife” option to the spell after the incident in question. He probably thought it very unlikely beforehand that such an attack could happen and then added this specific defensive option to the spell precisely because he was now painfully aware that the artifact existed in the village.

Eldor has contingencies, revealed later. Also, the look in his eye in the previous and following panels suggests he’s watching for such a thing. He was ambushed by that same knife before, and Lilith has suddenly lashed out at him not ten pages ago.
But… Lilith is at the bottom of a well, with no escape, not even taking of one’s own life. She’s near constantly watched, and can’t command her friends to kill her (in abject horror they would refuse, and might face consequences from Eldor.) She is at this point completely broken, and at the mercy of Eldor and his machinations for the time being.
Death or cake? No, books! Looking back I think that this would have been a great “stupid pajamas scene” for Eldor. If those are his pajamas, they look the same as all the other fashion in a medieval age. It’s late at night, but Lillith will sleep when she’s dead.
