My comment for this page from Gaia: sic mundus creatus est:Again and again, readers commented that the term “the First” is omitted from the titles of kings and queens who are the first with that particular name. Well, on Earth. But this is Gaia, where it usually isn’t omitted. But I ultimately decided against using a text box to explain it. You can’t litter your comic with text boxes that explain every miniscule detail of your setting.

Is staying standing considered rude in that situation?
Never mind the “the first” here, which can be overlooked. A much bigger issue here is the use of “vow” instead of “swear” here!

Saying “the First” can be seen as a prayer that the line will continue.

Yeah, it’s perfectly reasonable to have The First of Her Name in there, it’s done every now and then.
But you don’t really vow to do something, a vow is something you’ve sworn to dom
I like how Ilias carries himself here. He’s serious, supportive, unfazed, and it is clear that he is there to support Jael I. Which is fitting, since he is both her fiance and her representative, and is there at her service. It wouldn’t do for him to take the spotlight, nor for him to interrupt whatever the Queen wants him to be doing just because someone else shows up. And in this case, that would include helping her to stand with as much dignity as possible. His job is to help her look good, whatever that happens to require. Given how quick she is to adapt and how good a leader she is, that’s probably not a hard task, but Ilias seems to be the sort that would take that as an opportunity to help her do even better, rather than just slack off knowing she can handle it.
(It comes to mind that it’s rather like guide dogs. While they are working, they are supposed to be 100% focused on doing their assigned task, whatever that happens to be. They’re not there for pets or treats or fun or anything else. They’re not supposed to answer commands from anyone other than the person they are tending to, with very few exceptions)
In any case, chances are this is less of a surprise to him than it is to her. He’s been awake while she is unconscious, so he would be largely aware of the political currents and events. He has also been acting for her, so he may already be accustomed to people being respectful to her.
