Well, the phrase is a reference to how you put your hands on a steering wheel when driving. If you try REALLY HARD to picture Sandra as a steering wheel that Cloud puts his hands on, then 2 and 10 would be a bit too high for safety.

this is part of Larisa’s long term plan of making sure Cloud and Sandra get and stay together. Lar knows what going to happen to her body eventually, so she’s taking steps to make sure the people most important in her life are as happy as possible. Even though Larisa probably loves Cloud in her own way, she knows that him and Sandra are the best for each other.
Knowing Larisa she’s probably doing the same for Landon-giving him the best ‘first relationship’ she can. While I personally hope she and Landon stay together, I’ve got a feeling that Larisa is trying to give him a great relationship right now (judging from the subtext what kind of ‘picture’ she sent him for his birthday shows she’s a lot more comfortable with herself than most of her peers and wants Landon to recall as many good memories as possible) as she feels deep down it’s not the same as Cloud and Sandra and probably won’t last beyond high school/college.
Plus, Larisa is going to have to tell Landon eventually, even if they’re not dating at the time so he won’t be blindsided when it happens. I’m don’t really have any proof but I see Landon as the type who would start taking extra biology classes to focus on specifically helping Larisa and do whatever it took to try and save her.
